The facility offered 120 beds—seven below the county average during the period—and served an average of 108.9 residents.
The CMS gave Willow Ridge Nursing and Rehabilitation Center, LLC an overall rating of 4 out of 5, which surpasses the Louisiana nursing home average of 2.5.
Leslie Lakes Retirement Center led as Bienville County’s largest nursing home that quarter with 150 beds, while Ringgold Nursing and Rehabilitation Center, LLC followed in third place at 112 beds.
Willow Ridge Nursing and Rehabilitation Center, LLC has had $10,059 in fines and one penalty as of the first quarter of 2026.

U.S. Census Bureau estimates show the nation’s older adult population continues to rise as younger groups decline. From 2020 to 2024, Americans age 65 and older increased by 13%, while those under 18 declined by 1.7%. The older adult population in the U.S. has expanded considerably over the past 100 years, growing from 4.9 million in 1920 to more than 61 million by 2024.
A 2021 study found nearly half of Americans in this age group—about 28 million—may need long-term services and supports, with projections indicating more than a third could eventually require nursing home care.
“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education organization. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
